Apart from our acute clinics (see our home page for days/times), access to our services begins with a referral. Don’t stress – it’s a quick and easy process. Just complete the form below and we’ll be in touch.
You can refer yourself, or you can be referred by another organisation, agency, or medical centre. If you are referring someone other than yourself, you must have their permission to do so.
We review all referrals to ensure they fit within our service criteria. While we can’t promise to accept every referral, we will ensure everyone who reaches out is guided toward appropriate support – whether that’s with us or another service.
We aim to respond within 10 business days.

A Note About the Information We Collect
This referral form collects only the basic information we need to understand your situation and determine whether our services are a good fit.
If you proceed to engage with one of our services, we will collect more detailed information at that stage as part of your intake process.
We collect only information that is necessary and handle it in accordance with the Privacy Act 2020 and the Health Information Privacy Code.
Information provided in this form is used for referral processing and service coordination. It will not form part of a clinical record unless engagement proceeds.
Referral information is securely stored for administrative, audit, and service planning purposes. Referrals that do not proceed to engagement are retained for up to 24 months before being securely deleted, unless required for legal or safety reasons.

Medical Services Information
You can access our medical services as either a casual or enrolled patient.
If you wish to enrol with us for regular, ongoing healthcare, you will need to transfer from your current GP. A form of legal identification (e.g. birth certificate, driver’s licence, passport) will be required at enrolment.
If you wish to access medical services for a one-off consultation or short-term period, you may attend as a casual patient. Additional information such as full name, address, GP details, and next of kin will be collected at the time of your appointment.
